What N-522 means

RARC N-522 is an informational code from a secondary payer indicating the claim cannot be processed without the primary payer's explanation of benefits (EOB). This is a documentation issue, not a true denial — attaching the primary EOB resolves it.

Common causes

  • Secondary claim submitted without the primary EOB attached
  • Primary EOB faxed or mailed separately and not matched to the claim
  • Electronic crossover claim did not carry the necessary COB data

How to appeal N-522

Obtain the primary payer's EOB, attach it to a corrected claim or appeal, and resubmit to the secondary. If the claim should have crossed over electronically, contact the primary payer to confirm the crossover was triggered or submit manually with the EOB attached.
