What CO-B7 means
The claim was denied because the provider is not enrolled or credentialed with the payer for the service billed. Frequently a credentialing lag — the provider is in-network but processing hasn't completed.

- New provider not yet credentialed with the payer
- Provider credentialing application pending or lapsed
- Rendering provider NPI not linked to the billing entity
How to appeal CO-B7
Confirm the credentialing status with the payer's provider relations team. If credentialing is pending, request a retroactive effective date once approved. Attach proof of application submission and any provisionally-approved correspondence in the appeal.
